Tax Forms

Federal, state, and school district tax forms are typically available at the library from January through April. Library staff cannot assist patrons in the preparation of their taxes or tell them which forms to file. Many of the forms available at the library can also be found online through the websites below.  

Golden Buckeye Card Registration

Ohioans age 60 and above, as well as those individuals of any age who are disabled as defined by Social Security, are eligible to receive a Golden Buckeye Card. The Golden Buckeye Card is honored at businesses throughout the state and helps you to save money through discounts and other incentives. Generally a card is mailed to eligible individuals in the month of their 60th birthday, assuming those individuals have a drivers license or other form of state ID.

If you have not received a Golden Buckeye Card and thing that you should, you can fill out an application at the library and staff will gladly fax it to the Ohio Department of Aging so that you can receive your card as soon as possible. Please bring proof of age and/or proof of disability with you when you come to apply.

Voter Registration

The library is a great place to register to vote or to have your voter registration information updated. We have voter registration forms on hand and will even send your completed registration form to the appropriate county board of elections.

The library has a variety of forms as well as local information available in our INFOmation Center which is located in the Noel Shaffer Reading Room of the library.
